ORS is an independent research agency that carries out the Health Assessment Survey on behalf of the Department for Work and Pensions in England, Scotland and Wales, and on behalf of the Department for Communities in Northern Ireland.

Each month, ORS speaks to around 1,500 people who have had a Health Assessment to ask them about their assessment and the way it was carried out by the provider.

The Department for Work and Pensions and the Department for Communities use the feedback from the survey to help them provide the best service to people that apply for benefits in future, so we do hope that you will have time to share your feedback.

Who are you?

We are Opinion Research Services Ltd, a limited company registered in England and Wales to undertake Market Research and Public Opinion Polling and our registered company number is 02904006.

What is the survey about?

The survey questions cover a range of topics about your health assessment and the way in which it was carried out by the provider, as well as some information about you.

Whether or not you choose to take part and any answers that you give to the survey questions will not influence the outcome of your health assessment or affect any benefit payments you receive now or in the future.

Taking part in the survey will help to shape the service that is provided and improve the experience for people that apply for benefits in future, so we do hope that you will have time to share your feedback.

Who is carrying out the survey?

Opinion Research Services (ORS) is carrying out the survey on behalf of the Department for Work and Pensions in England, Scotland and Wales, and the Department for Communities in Northern Ireland.

ORS is an independent social research agency that adheres to the Market Research Society Code of Conduct. This validates that we work to the highest professional standards.

Why did you choose me to take part?

Your name was chosen at random from the list of everyone that has had a health assessment this month.

How did you get my details?

You provided your details to the Department for Work and Pensions or the Department for Communities in case they needed to contact you, which included asking you for feedback.

Your contact details have been shared with the ORS survey team for them to get in touch with you to ask about the health assessment.

Will my information be safe and secure?

All of the data that ORS processes is always safe and secure. We will treat all of your information in strictest confidence under UK General Data Protection Regulation. You can read our Privacy Notice for further information about how we manage data.

Your details will only be used by ORS for this survey and you will not receive any junk mail or marketing calls as a result of taking part in the survey.

We have an Information Security Management System which is accredited to ISO 27001. This covers the security of our IT systems, the physical security of our premises and the operational measures that are in place through policies, procedures and working practices. Any new systems or software that we develop are designed with privacy in mind.

Our IT systems are also accredited under the Cyber Essentials scheme that is overseen by the National Cyber Security Centre.

Who will see my answers?

The answers that you give to the interviewer will be seen by researchers from ORS that are working on the survey.

If you live in England, Scotland or Wales, your answers will be shared with researchers from the Department for Work and Pensions to help them monitor the performance of the health assessment providers. The researchers will be able to identify you from the data, as this will help them to identify any improvements needed. If you live in Northern Ireland, your answers will be shared with researchers from the Department for Communities to help them monitor the performance of the health assessment providers in Northern Ireland. How do I take part?

Enter your Reference Code in the box at the top of this page and you can then click to take part online or let us know what time to call.

You can also take part by calling Freephone 0800 324 7005 before 8:00pm Monday to Friday and before 4:00pm on Saturday.  If you call after this time, you may need to leave a message and we will call you back as soon as possible.

If you don’t get in touch or tell us when to call you, one of our interviewers will telephone you in the next week or so.

Can I take part in another language?

Yes.

The online questionnaire is only available in English and Welsh, but if you need to take part in another language you can email surveys@ors.org.uk with the Reference Code that we sent you, your contact details and details of the language that you need and we will arrange for you to take part.

Do I have to take part?

Taking part in the survey is voluntary.  We hope that you will take part as the Department for Work and Pensions and the Department for Communities rely on the people that are chosen to give them feedback about the services that are provided.

If you do not want to take part, you can enter your Reference Code at the top of this page and then click to withdraw from the survey.
